We have considered all your feedback from the last online report and have, we believe, improved the survey greatly in terms of its effectiveness and efficiency. We have also attempted to pre-populate information fields with the responses from your last online report. As you will see, we were not able to pre-populate with all your previous information but we've done our best and will continue to work to improve this and make it as easy as possible for you.
Note that a PDF version of the report is available for your advance collection of data. The electronic survey is MANDATORY to enable the data to be analyzed electronically.
Should you begin the survey and want to come back to it, you may simply revisit the link.
Also, after you've completed the survey, we will send you an electronic copy of your submission. Unfortunately, there is no print function with the online report tool for you to do this yourself.
